Following every Adopt a School programme both the club/centre and school representatives are required to complete an end of programme evaluation.

The evaluations are usually completed online but while the website is being redeveloped the evaluations are available as a hard copy. Please download the appropriate evaluation below and once complete please return to the FIA by email to Lindsay@fia.org.uk, fax to 020 7202 4701 or by post to Adopt a School, FIA, 4th Floor, 61 Southwark Street, London, SE1 0HL to be inputted.

Please note that it is the responsibility of the club/centre co-ordinator to make sure that both the club/centre and school evaluations are completed and returned to the FIA.
